Malfunctioning grenade launcher

As you progress during a campaign in the game the grenade launcher starts to malfunction. It will fire with a shotgun sound and NO effect. Has happened repeatedly in 3 different campaign games.

geever:
Maybe you loaded flechette grenades which are pretty like using grenade launcher as a shotgun. You need to aim to the enemy with it, not the ground like with HIT grenades.
